Register First Time Users
Follow these simple steps to create a new account in our Patient Portal
To Register:
2) Click on Not Registered?
3) Enter your email address. Use the same email we have on file to sign up. If the email does not match the one we have on file, this will not work. If you are having trouble, give the office a call to verify your email address.
4) You will receive an email with a VERIFICATION CODE. Enter this code in the appropriate space and enter your birth date. Click NEXT. If you are having trouble finding the email, make sure to check your spam folder.
5) Create a secure password and security question for your account and click UPDATE.
6) All done! You will now be redirected to your home screen. You can click MY HEALTH RECORDS to view your exam data and records. The login and password you have created can be used to view future exam records at your convenience.
